Formly — Form Builder & Submissions (Laravel)

Formly is a self-hosted form builder built with Laravel 11 and MySQL. Create unlimited forms with a visual field builder, share a public link, and collect & manage submissions with CSV export — no SaaS fees, no monthly lock-in. Tailwind via CDN, custom authentication, no npm/Vite build step.

The builder supports nine field types (text, email, phone, number, paragraph, dropdown, multiple choice, checkboxes, date) with required flags and choice options. Every active form gets a clean hosted page at /f/{slug} with server-side validation and a thank-you page. The submissions inbox tracks read/unread, shows full detail and exports to CSV.
