Formly — Form Builder & Submissions (Laravel)

Formly requires PHP 8.2+ with a MySQL database and Composer, as it is built on Laravel 11. There is no Node.js, npm, or Vite build step needed — Tailwind CSS is loaded via CDN, so a standard LAMP or LEMP stack is all you need.

Formly is a fully self-hosted PHP script — you download it, deploy it on your own server, and own it outright. There are no monthly SaaS fees, no external accounts required, and no data sent to third-party services.

The visual builder includes nine field types: text, email, phone, number, paragraph, dropdown, multiple choice, checkboxes, and date. Each field can be marked as required, and choice-based fields (dropdown, multiple choice, checkboxes) support custom option lists.

All submissions are stored in your MySQL database and appear in a built-in submissions inbox that tracks read/unread status with a full detail view. You can export every response for any form to a CSV file with a single click — no third-party tools needed.

Yes — each form has its own settings panel where you can configure the button label, thank-you/success message, a notification email address, and a live/draft toggle. This lets you take a form offline or update its messaging at any time without deleting it.

Formly ships with a custom-built authentication system (no Laravel Breeze or Jetstream dependency), so it works out of the box without extra setup. The clean Laravel 11 codebase, Tailwind CDN styling, and included database seeder (admin account plus two demo forms and submissions) make it straightforward to extend or rebrand for a client project.
